Malnutrition in elderly patients with diabetes mellitus and hypertension: Geriatric syndromes and calf circumference

Abstract

Malnutrition is a common comorbidity among the elderly, particularly in individuals with type 2 diabetes mellitus (T2DM) and hypertension (HTN). This condition directly affects metabolism through mechanisms including chronic hyperglycemia, low-grade inflammation, increased protein catabolism, and insulin resistance. Malnutrition is associated with numerous adverse consequences including prolonged hospitalization, increased complications, higher readmission rates, and increased short- and long-term mortality in hospitalized elderly patients. A study of 296 patients aged ≥ 60 years old with concurrent T2DM and HTN receiving inpatient care revealed that 6.8% were underweight, 47.3% were overweight according to BMI; 33.8% were at risk of malnutrition, 15.2% had malnutrition according to MNA-SF, and 38.5% (114/296) had malnutrition according to GLIM criteria. The prevalence of malnutrition in elderly patients with T2DM and HTN is at a high level. Elderly patients with reduced calf circumference, combined with weakness, depression, and physical inactivity should be screened for nutritional status.
