Clinical characteristics of epicanthal folds in surgically treated patients at the National Eye Hospital

Abstract

This study describes the clinical characteristics of medial epicanthal folds in 31 patients (62 eyes) examined at the Vietnam National Eye Hospital from October 2023 to July 2025. Patients were evaluated for epicanthal fold severity using the Taylor classification, palpebral fissure length, inner canthal distance, levator muscle function, and associated ocular abnormalities. The results showed that moderate to severe epicanthal folds were predominant, with grade 3 being the most common (51.6%). The mean palpebral fissure length was shorter than normal values, while the inner canthal distance was increased, with a high proportion of eyes showing an inner canthal distance–to–palpebral fissure length ratio ≥ 1.4 (77.4%). Common associated conditions included absent or poorly defined eyelid creases, epiblepharon, and ptosis. A statistically significant association was observed between epicanthal fold severity and both palpebral fissure length and inner canthal distance. These findings provide useful anatomical and clinical information to support assessment and guide management strategies for patients with medial epicanthal folds.
