Assessment of high fluorescent lymphocyte count value (HFLC) in the diagnosis and monitoring of patients with Dengue fever at Duc Giang General Hospital

Abstract

The study describes the count and percentage of HFLC in dengue fever patients at Duc Giang General Hospital, and analyzes the variation of HFLC across disease stages and severity levels. This was a retrospective descriptive study conducted on 190 dengue fever patients at Duc Giang General Hospital (October 2024 – October 2025). The control group consisted of 160 healthy individuals. The HFLC parameter was measured using the Sysmex XN-1000 analyzer. Statistical analysis was performed using SPSS version 20.0. The median age was 32 years old (IQR 24 – 44), predominantly males at 62.6%. Among dengue patients, 38.9% had no warning signs, 51.1% had warning signs, and 10.0% had severe dengue. HFLC increased continuously from the time of admission, peaking on day 5 (HFLC# 0.91 G/L; HFLC% 21.7%), coinciding with the nadir of platelet count (29 G/L). At the time of diagnosis, an HFLC# threshold of ≥ 0.11 G/L yielded an AUC of 0.941 (95% CI: 0.918 – 0.964), with a sensitivity of 84.2% and specificity of 91.3%. The combination of HFLC# and platelet count further improved the AUC to 0.963. Among 97 patients with warning signs, 15 patients (15.5%) progressed to severe dengue at a median of day 6; an HFLC# threshold of ≥ 0.56 G/L at admission demonstrated an AUC of 0.874 with a negative predictive value (NPV) of 96.6%, while an HFLC# threshold of ≥ 1.18 G/L on the day immediately preceding clinical deterioration yielded an AUC of 0.948 with an NPV of 97.5%.
