Prevalence and associated factors of prolonged active-phase labor: A cross-sectional study of 504 pregnant women

Abstract
Prolonged active labor is an important contributor to increased obstetric interventions and adverse maternal and neonatal outcomes; therefore, determining its prevalence and associated factors has practical implications for labor management and prevention of unfavorable outcomes. A cross-sectional study was conducted on 504 women in active labor at Thuan My TDM Hospital from May 2025 to January 2026. Prolonged labor was defined as cervical dilatation of at least 4 cm with a dilatation rate of less than 1 cm per hour or a labor curve deviating to the right of the alert line on the 2002 WHO partograph. The prevalence of prolonged active labor was 12.1%, corresponding to 61 of 504 cases. Multivariable analysis showed that maternal anxiety, cervical consistency, and uterine contraction characteristics, including contraction frequency per 10 minutes and Montevideo units, were independently associated with prolonged active labor, with p < 0.05. The prevalence of prolonged active labor was clinically noteworthy, and early identification of associated factors may help optimize labor monitoring and management.
