Clinical case report of asymmetric bilateral lumbar hernias and literature review

Abstract

Lumbar hernia is a rare condition. The concurrent occurrence of superior (Grynfeltt) and inferior (Petit) lumbar hernias asymmetrically in the same patient is extremely rare. We report the case of a 68-year-old female with a history of polyarthritis, hypertension, coronary stent placement, and Cushing syndrome. Computed tomography revealed not only a left superior lumbar hernia but also a concurrent right inferior lumbar hernia. The patient was treated using the totally extraperitoneal endoscopic technique (eTEP). The surgery was successfully performed in 75 minutes. A mesh was placed to cover the entire left hernia defect after reducing the herniated contents and closing the hernia neck. The postoperative course was stable; the patient was discharged after 7 days and showed no recurrence after 1 month. The eTEP technique is a minimally invasive, safe, and effective intervention that provides durable abdominal wall reconstruction for complex lumbar hernias.
