Effect of conscious sedation with propofol TCI on the analgesia nociception index (ANI) during fiberoptic intubation in patients with maxillofacial abscess

Abstract
Awake fiberoptic intubation is often associated with pain and marked discomfort, resulting in poor patient cooperation and potentially causing significant increases in heart rate and blood pressure. This study aimed to evaluate the effect of conscious sedation with propofol target-controlled infusion (propofol TCI) on the Analgesia Nociception Index (ANI) during fiberoptic intubation in patients with maxillofacial abscess and trismus. This was a secondary analysis of a randomized controlled interventional study involving 102 patients undergoing surgery for maxillofacial abscess with trismus. Patients were divided into two groups: Group 1 (n = 51) received conscious sedation with propofol TCI combined with topical anesthesia of the upper airway; Group 2 (n = 51) received topical anesthesia of the upper airway without sedation. Results showed that when the fiber optic scope passed through the glottis and immediately after intubation, ANI decreased significantly from baseline in both groups (p < 0.05). However, ANI values were significantly higher in Group 1 than Group 2 (p < 0.05). Conclusion: Conscious sedation with propofol TCI may help maintain higher ANI values compared with no sedation during fiberoptic intubation in patients with maxillofacial abscess and trismus.
