Sarcomatoid renal cell carcinoma with rib metastasis: A rare case report and review of the literature

Abstract

Sarcomatoid renal cell carcinoma (sRCC) is a rare form of dedifferentiated carcinoma with a high metastatic rate and poor prognosis. Common sites of metastasis include lymph nodes, lung, liver and bone. A case of sRCC with unusual rib metastasis in a 52-year-old male was reported. The patient was hospitalized because of pain in his right shoulder. CT-scanner showed a tumor that measured 36x37 mm in the mid-pole of the right kidney and a focal bone resorption lesion on the right 6th rib. The patient had a rib biopsy and a kidney biopsy with a diagnosis of sRCC with rib metastasis; he then had a complete right nephrectomy. Histologically this tumor consisted of a clear cell renal cell carcinoma component combined with a sarcoma-like component with malignant spindle cells. Postoperative diagnosis was seen in a clear cell renal cell carcinoma with sarcomatoid differentiation with rib metastasis, grade IV. SRCC is relatively rare and has poor prognosis. The tumor is composed of two histological components, epithelium and sarcoma, which can make diagnosis difficult on preoperative biopsy.
