Magnetic resonance imaging features of parapharyngeal space tumors: A 10-case series with histopathologic correlation

Abstract
This study aimed to describe the magnetic resonance imaging (MRI) features of parapharyngeal space tumors with histopathologic correlation, while emphasizing the value of lesion localization in preoperative assessment. We retrospectively reviewed 10 cases that underwent surgery at the National Otorhinolaryngology Hospital of Vietnam between January 2023 and March 2026. Collected data included clinical findings, lesion location, T1W/T2W signal characteristics, diffusion features (DWI/ADC), enhancement pattern, tumor margin, invasion, cervical lymph nodes, and histopathology. The median age was 48.5 years old, with an equal gender distribution. Seven tumors were located in the prestyloid compartment, two in the poststyloid compartment, and one extended into both compartments. Histopathology showed seven benign and three malignant tumors. Pleomorphic adenoma was the most common benign tumor, whereas mucoepidermoid carcinoma was the most common malignant tumor. Benign lesions usually had well-defined margins and no abnormal cervical lymph node, whereas malignant lesions more often showed infiltrative margins, heterogeneous enhancement, more marked diffusion restriction, and/or invasion of adjacent muscles. MRI played a useful role in lesion localization, suggesting tissue of origin, and supporting preoperative surgical planning.
