Effectiveness and safety of ramucirumab -containing regimens in advanced gastric cancer at National Cancer hospotal

Abstract
To evaluated the efficacy, safety, and selected prognostic factors of Ramucirumab-containing regimens in 51 patients with metastatic or recurrent metastatic gastric/gastroesophageal junction adenocarcinoma treated at Vietnam National Cancer Hospital from January 2025 to May 2026. Ramucirumab was mainly used as second-line treatment, accounting for 40/51 cases (78.4%). The most common regimen was Ramucirumab plus Paclitaxel, used in 37/51 patients (72.5%). The objective response rate was 3.9% in the overall cohort and 4.5% among evaluable patients; the corresponding disease control rates were 29.4% and 34.1%, respectively. Median progression-free survival (PFS) and overall survival (OS) were 3.12 and 7.98 months, respectively. Prior primary tumor surgery was an independent prognostic factor associated with a higher risk of disease progression (HR = 2.20; 95% CI: 1.01 - 4.80; p = 0.048), whereas male sex was associated with a lower risk of death (HR = 0.21; 95% CI: 0.06 - 0.70; p = 0.011). Regarding safety, 94.1% of patients experienced at least one adverse event or symptom during treatment; grade 3-4 adverse events occurred in 25 patients (49.0%), most commonly grade 3-4 leukopenia/neutropenia (25.5%). No adverse event-related deaths were recorded. Ramucirumab-containing regimens showed disease control in a subset of patients in routine clinical practice, with a toxicity profile that was monitorable and manageable.
