The value of serum M2BPGi concentration in the diagnosis of hepatocellular carcinoma

Abstract

To determine serum M2BPGi cutoff values for the diagnosis of hepatocellular carcinoma (HCC) a cross-sectional study was conducted on 120 patients with hepatitis B virus (HBV) infection at Bach Mai Hospital from January 2025 to January 2026; there were 60 HCC patients and 60 chronic hepatitis B (CHB) patients. All patients underwent serum Mac-2 binding protein glycosylation isomer (M2BPGi) testing. The HCC group was predominantly male, aged over 60 years old, and exhibited significantly higher degrees of liver fibrosis and cirrhosis compared to the CHB group (p < 0.001). 45% of the HCC patients had M2BPGi levels at the 2+ grade, which was significantly higher than the 3.3% observed in the CHB group (p < 0.001). M2BPGi demonstrated high diagnostic performance for HCC with an area under the curve (AUC) of 0.854 (p < 0.001). At a cutoff index (C.O.I) ≥ 0.8, the sensitivity, specificity, and accuracy were 91.7%, 51.7%, and 71.7%, respectively. At a C.O.I ≥ 1.7, the sensitivity was 61.7%, the specificity was 88.3%, and the accuracy reached 75.0%.
