Mobility, activities of daily living and associated factors in patients with osteoporotic vertebral compression fractures

Abstract

This cross-sectional descriptive study enrolled 200 patients with osteoporotic vertebral compression fractures (T-score ≤ −2.5) at the Spine Clinic, Military Hospital 108 (January–August 2025). Basic activities of daily living (ADL) were assessed by Barthel Index and instrumental ADL (IADL) by Lawton IADL scale; multivariable linear regression identified independent predictors. Mean age was 69.97 ± 8.11 years old; 92.0% were female. Mean Barthel Index was 82.63 ± 16.95; 80.5% had varying degrees of dependency; transferring (77.5%), stair climbing (66.5%), and walking on flat surfaces (52.0%) were most impaired. Mean Lawton IADL was 5.85 ± 2.07; 66.0% had some degree of IADL impairment; housekeeping (69.0%), transportation (68.0%), and laundry (67.5%) were most affected. Independent predictors for Barthel Index (R² = 0.660) were age (B = −0.40; p = 0.037), BMI (B = −0.61; p = 0.017), and pain VAS (B = −7.95; p < 0.001). Independent predictors for Lawton IADL (R² = 0.548) were age (B = −0.11; p < 0.001), pain VAS (B = −0.47; p < 0.001), and number of fractured vertebrae (B = −0.29; p = 0.041). Pain intensity was the strongest predictor in both models.
