Assessment of serum resistin levels and their association with disease severity in patients with primary knee osteoarthritis

Abstract
Adipokine Resistin is adipokine secreted primarily by macrophages and monocytes, playing an important role in the activation of inflammatory responses and the process of articular cartilage destruction. This study was conducted to investigate the characteristics of serum resistin levels and their association with disease severity in patients with primary knee osteoarthritis. A cross-sectional descriptive study was conducted on 79 patients with primary knee osteoarthritis treated at Binh Thuan General Hospital from August 2025 to February 2026. The results showed that most patients were in stage II (55.7%) and stage III (24.0%), while stages I and IV were lower at 15.2% and 5.1%, respectively. In stage I, the median serum resistin level was 383.85 pg/mL (IQR: 321.56 - 844.48), increasing to 726.95 pg/mL (IQR: 433.32 - 1168,83) in stage II and reaching 1981.92 pg/mL (IQR: 1883.20 - 2882.09) in stage III, and peaking at 2217.31 pg/mL (IQR: 1253.82 - 3184.73) in stage IV (p < 0.001). After adjustment for potential confounders, resistin was independently associated with disease severity, OR = 1.30 (95% CI: 1.02 - 4.20; p = 0.041). These findings indicate that serum resistin levels increase with the severity of knee osteoarthritis and serve as an independent factor associated with severe disease.
