Surgical management of pediatric renal tumors with inferior vena cava thrombus: Three cases

Abstract
The study reports the treatment outcomes of three pediatric renal tumors with inferior vena cava thrombus, including one case extending into the right atrium. All three patients, aged 1 to 6 years old, received neoadjuvant chemotherapy before surgery, followed by radical nephrectomy, inferior vena cava control, longitudinal venotomy with complete tumor thrombectomy, and primary venous repair. One patient with thrombus extension into the right atrium was successfully managed with the support of cardiopulmonary bypass. Postoperatively, two patients with Wilms tumor remained stable after 10 - 14 months of follow-up, whereas one patient with malignant rhabdoid tumor of the kidney developed early recurrence and metastasis after only 3 months. These initial results highlight the important role of neoadjuvant chemotherapy and suggest that venotomy for tumor thrombectomy followed by primary venous repair is feasible in this group of patients.
