Association between EGFR mutations subtype and bone metastasis in metastatic non-small cell lung cancer

Abstract
The purpose of this study was to investigate the association between subtype EGFR mutation and bone metastasis in patients with non-small cell lung cancer (NSCLC). We retrospectively analyzed the results of 1,015 patients with NSCLC who had distant metastases at the time of diagnosis and underwent EGFR mutation testing between January 2015 and January 2025. Multivariable logistic regression models were used to assess the independent associations between EGFR mutation status, EGFR mutation subtypes, and the presence of bone metastasis. Bone metastases were present in 45.8% of patients. EGFR mutations were more frequent in patients with bone metastases than in those without bone involvement (46.7% vs. 37.1%, p = 0.002). Both exon 19 deletion and the L858R mutation were independently associated with bone metastasis (OR = 1.52 (CI 95%: 1.1 – 2.08), p = 0.01 and OR 1.96 (CI 95%: 1.31 – 2.9), p = 0.001, respectively). EGFR mutations, particularly exon 19 deletions and the L858R mutation, are independently associated with the presence of bone metastasis at diagnosis in patients with NSCLC.
