Evaluation of the results of surgical treatment of internal fixation with screw plates for closed tibial plateau fractures at Hanoi Medical University Hospital

Abstract

The objective of the study was to evaluate the results of surgical treatment of internal fixation for closed tibial plateau fractures with screw plates. This is a descriptive study of 33 patients with tibial plateau fractures underwent surgery of internal fixation with screw plates at Department of Orthopedic Trauma and Sports Medicine - Hanoi Medical University Hospital. The period was from January 2019 to February 2021. The results showed that the mean age was 44.4 ± 14.3 years, youngest was 17 years old, oldest was 81 years old, the most common age was 20 - 40 years old (48.5%). 32/33 patients healed the first wound (96.96%), only 01/33 patients had late healing. Almost all of patients had good results; 19/33 patients were tight fracture (57.58%) and no articular was compressed (57.58%), 31/33 patients had no axis deformity or axis deformity < 10°. However, there were also a great number of patients who still have depression ≤ 5mm (42.42%) and there were two patients with axis deformity > 10° (6.06%). The percentage of patients with very good results according to Rasmusen's X-ray at grades I, II, III, IV was higher than that of grade V, VI; the difference was statistically significant with p < 0.05. When assessing function according to IKDC scale, 45.5% had very good results, 30.3% had good results, 18.2% had moderate results and 6% had poor results. The proportion of patients with very good results according to IKDC function in grades I, II, III, IV was higher than that of grade V, VI, especially patients in grades I, II, III, IV; no one had poor results. The difference was statistically significant with p < 0.05. Internal fixation surgery with screw plates is an effective treatment method for patients with closed tibial plateau fractures at Hanoi Medical University Hospital.
