Evaluation of neuropathic pain using the DN4 questionnaire and associated factors in patients with primary knee osteoarthritis

Abstract

A cross-sectional study was conducted to describe the characteristics of neuropathic pain using the DN4 questionnaire and to analyze several associated factors in 146 patients with primary knee osteoarthritis (according to the 1991 ACR criteria) at Bach Mai Hospital from December 2025 to April 2026. The results showed that the prevalence of patients with neuropathic pain according to the DN4 questionnaire is 45.9%. Commonly reported symptoms included: electric shocks (56.8%), painful cold (55.5%), and pain brushed or increased by light touching (53.4%). Univariate analysis revealed significant associations with neuropathic pain (p < 0.05) for: age > 70 years (OR = 3.2), disease duration ≥ 12 months (OR = 4.8), VAS score ≥ 7 (OR = 10.7), radiographic stage III, IV (OR = 5.6), and total WOMAC score (OR = 1.1). In multivariate regression analysis, the total WOMAC score was the only independent predictor significantly associated with neuropathic pain (aOR = 1.2; 95% CI: 1.1 – 1.3; p < 0.001). The results indicated that neuropathic pain accounts for a high proportion of patients with primary knee osteoarthritis. The total WOMAC score serves as an independent predictor, highlighting the need for early screening of neuropathic pain in patients with high WOMAC scores in clinical practice.
