Factors associated with disease control in patients with chronic spontaneous urticaria not completely controlled by High-Dose H1-antihistamines

Abstract

An ambispective study was conducted on 250 patients with chronic spontaneous urticaria (CSU) who failed to achieve complete disease control after 4 weeks of treatment with maximum-dose (up to fourfold standard dose) of the second-generation H1 antihistamines (sgAH1s Factors associated with poor disease control (UCT < 12) were investigated. The results showed that the rate of poor disease control among CSU patients who did not achieve complete control with fourfold-dose sgAH1s was 53.6%. Compared with the well-controlled group, patients with poor disease control under high-dose sgAH1 therapy were older, had higher baseline UAS7 scores, and lower total IgE levels (p < 0.05). Multivariable logistic regression analysis demonstrated that low total IgE levels (OR = 4.54; p = 0.02) and higher baseline UAS7 scores (OR = 1.04; p = 0.03) were independently associated with poor disease control among CSU patients who failed to achieve complete control with high-dose sgAH1. The model demonstrated modest discrimination (AUC = 0.668). Identification of these factors may help recognize CSU patients who are more likely to experience suboptimal disease control with high-dose sgAH1 therapy and may provide additional clinical context for future studies on individualized management strategies.
