Viêm mũi xoang thai kỳ: Tổng quan tường thuật về chẩn đoán phân biệt và an toàn điều trị

Nghẹt mũi khi mang thai có cơ chế đa dạng gồm viêm mũi thai kỳ, viêm mũi dị ứng và viêm mũi xoang nhiễm khuẩn; phân biệt ba thực thể này quan trọng để chọn điều trị nhưng thiếu hướng dẫn cụ thể tại Việt Nam. Tổng quan này tổng hợp bằng chứng về dự đoán, hiệu quả và an toàn các biện pháp điều trị viêm mũi xoang ở phụ nữ mang thai, theo phương pháp tổng quan tường thuật theo SANRA, tìm kiếm hệ thống trên PubMed, Cochrane và Embase, đánh giá chất lượng theo GRADE. 38 nghiên cứu đủ tiêu chuẩn được đưa vào phân tích với chứng cứ từ thấp đến trung bình. Dấu hiệu gợi ý nhiễm khuẩn đáng tin cậy nhất gồm dịch mủ, đau vùng mặt, giảm khứu giác và triệu chứng kéo dài > 10 ngày. Nước muối và budesonide có hồ sơ an toàn tốt nhất trong thai kỳ. Kháng sinh chỉ nên dùng khi có bằng chứng nhiễm khuẩn. Viêm mũi xoang ở thai phụ cần đánh giá có hệ thống, điều trị theo thang bậc và dữ liệu tiến cứu tại Việt Nam để xây dựng hướng dẫn thực hành.
