Characteristics of infectious spondylodiscitis at the Department of Rheumatology - Bach Mai Hospital

Abstract
Infectious spondylodiscitis is a serious infection characterized by nonspecific clinical manifestations and the diagnosis is often delayed. This study was conducted on 111 patients diagnosed with infectious spondylodiscitis at the Department of Rheumatology, Bach Mai Hospital from August 2025 to March 2026 to describe the clinical and paraclinical characteristics and identify the causative microorganisms. The mean age was 58.8 ± 15.0 years old, with a male-to-female ratio of 1:3; 62.2% of patients had comorbidities and 43.2% had a history of spinal medical interventions. Spinal pain was present in 100% of patients, fever in 52.3%, and neurological symptoms in 32.4%. On MRI, bone marrow edema was observed in 99% of patients, disc involvement in 79.8%, and the “mirror sign” in 78.6%. Elevated CRP levels were found in 93.5% of patients, while leukocytosis was observed in 48.6%. Positive blood cultures was higher than spinal specimen cultures (39.0% vs 19.4%). Staphylococcus aureus was the most common causative pathogen, while tuberculosis still accounted for a considerable numbers of cases.
