Effects of electrical impedance tomography-guided optimal PEEP setting on selected hemodynamic parameters in surgical ARDS

Abstract

This study evaluated the hemodynamic effects of EIT-guided PEEP titration in patients with moderate-to-severe ARDS related to trauma or postoperative conditions. This randomized controlled trial was conducted at Viet Duc University Hospital from May 2025 to May 2026. A total of 125 patients were analyzed, including 63 patients in the EIT group and 62 patients in the control group. PEEP was set higher in the EIT group on Days 2 and 3. No between-group difference in temporal trends was observed for heart rate, systolic blood pressure, diastolic blood pressure, mean arterial pressure, arterial lactate, norepinephrine use, or norepinephrine dose among observations with norepinephrine administration. Research showed that EIT-guided PEEP titration modified the PEEP strategy without showing a clear adverse hemodynamic signal.
