Outcomes of tympanoplasty with ossicular chain reconstruction in patients with chronic suppurative otitis media without cholesteatoma

Abstract
This study aimed to describe the clinical and paraclinical characteristics and evaluate the outcomes of tympanoplasty with ossicular chain reconstruction in patients with chronic suppurative otitis media without cholesteatoma. A descriptive study was conducted on 25 patients who underwent surgery between 2021 and 2025 and were followed for at least 12 months postoperatively. The mean age was 42.5 ± 15.3 years old, with 56.0% females. The most common symptoms were otorrhea (96.0%) and hearing loss (92.0%). Large tympanic membrane perforations accounted for 64.0% of cases, while mixed hearing loss was observed in 88.0%. The mean preoperative air-conduction pure-tone average (PTA) and air–bone gap (ABG) were 51.8 ± 15.1 dB and 33.6 ± 10.2 dB, respectively. Long-process incus erosion was the most common ossicular lesion (84.0%). Autologous grafts and partial ossicular replacement prostheses (PORP) were used in 92.0% of patients. No recurrent tympanic membrane perforation was observed postoperatively. Mean PTA improved to 40.3 ± 13.6 dB and mean ABG decreased to 21.6 ± 9.9 dB. The mean improvements in PTA and ABG were 11.5 ± 18.7 dB and 12.0 ± 15.6 dB, respectively (p = 0.005 and p < 0.001). Subjective hearing improvement was clearly reported by 60.0% of patients. Tympanoplasty with ossicular chain reconstruction achieved favorable anatomical outcomes and improved hearing function in most patients.
