Assessment of bleeding, pain and wound healing after tooth extraction in patients on antiplatelet therapy at school of dentistry and Bach Mai hospital

Abstract
This prospective analytical observational study aimed to evaluate the outcomes of tooth extraction in patients receiving antiplatelet therapy at the School of Dentistry and Bach Mai Hospital during 2025 – 2026. A total of 38 patients on antiplatelet medications with indications for tooth extraction were enrolled. Within the first 24 hours after extraction, 57.9% of patients experienced mild self-limiting bleeding, while 18.4% required additional hemostatic intervention. No significant differences in bleeding outcomes were observed among the different antiplatelet therapy groups (p > 0.05). The number of extracted teeth was identified as a significant risk factor for bleeding requiring intervention (OR = 6.39; p = 0.012). The mean Visual Analog Scale (VAS) pain score decreased from 4.02 ± 0.62 immediately after extraction to 0.91 ± 1.05 on postoperative day 7. The mean Landry Wound Healing Index and Early Healing Score (EHS) were 10.53 ± 0.95 and 5.55 ± 0.77, respectively. These findings suggest that no major bleeding events were observed in this study when antiplatelet therapy was maintained and appropriate local hemostatic measures were applied..
