Flanged intrascleral-fixated intraocular lens tilt and the correlastion with ocular residual astigmatism

Abstract
While intraocular lens (IOL) tilt is a known complication, its impact on visual acuity and ocular residual astigmatism (ORA) remains not fully elucidated. This cross-sectional descriptive study evaluated 32 eyes post-vitrectomy with scleral-fixated IOLs. An IOL was defined as tilted when the total tilt exceeded 7°. Tilted IOLs were observed in 34.38% (11/32) of eyes, with median vertical, horizontal, and total tilt angles being 3.25°, 2.40°, and 4.87°, respectively. The median ORA was 0.63D. Uncorrected visual acuity demonstrated no significant association with total tilt (r = 0.162, p = 0.373). Conversely, ORA exhibited a positive correlation with total tilt (r = 0.550, p = 0.001), governed by the linear regression: log(ORA) = −0.330 + 0.026 × (Total tilt). Conclusively, the majority of scleral-fixated IOLs maintained a tilt angle below 7°. IOL tilt was correlated with ORA, whereas no correlation was found with the patients’ visual acuity.
