25. Total laparoscopic pancreaticoduodenectomy: The first case report at Bach Mai Hospital and literature review on indications and results

Abstract
Laparoscopic surgery is increasingly replacing classic open surgery. Total laparoscopic pancreaticoduodenectomy (TLPD) has also gradually performed at many centers in the world and Vietnam, but the number is still modest. We hereby report the first successful total laparoscopic pancreaticoduodenectomy case performed at Bach Mai Hospital and review the medical literature on indications and results. This is a 64-year-old female patient, diagnosed with ampulla of Vater cancer (T3N0M0) underwent total laparoscopic unbloc-pancreaticoduodenectomy. Surgical time was 540 minutes, with blood loss of 150ml. Patient had postoperative intra-abdominal bleeding on day 4, was stabilized with medical treatment. The patient was discharged on the 23rd day with no other complication. Postoperative pathological result: adenocarcinoma, metastasis in 4/32 lymph nodes (T3bN2M0). This case shows that a total laparoscopic pancreaticoduodenectomy can be performed safely and has good oncological result with resectable periambullary tumors.
