Evaluation of peripheral blood eosinophil stability in patients with chronic obstructive pulmonary disease at the national lung hospital

Abstract

Peripheral blood eosinophil (EOS) count is a biomarker recommended by GOLD 2024 to guide decisions regarding usage of inhaled corticosteroids (ICS) in patients with chronic obstructive pulmonary disease (COPD). This study was conducted on 213 stable COPD outpatients at the National Lung Hospital; patients were monitored over 12 months to evaluate EOS stability and identify associated factors. Results demonstrated a positive correlation between two time points (rho = 0.5755; p < 0.001), with moderate stability (ICC = 0.52). After 12 months, 57.8% of patients remained in the same EOS category, while 42.2% shifted groups; notably, 37.5% of patients with EOS ≥ 300 cells/µL declined below the threshold. The considerable variability of EOS limits the value of a fixed cut-off for ICS indication. This study supports EOS as a potential marker for individualized COPD management; however, repeated measurements and integration with other clinical factors are necessary to optimize ICS treatment strategies.
