Abstract

This study aimed to assess the current situation of antihypertensive drug use in Vietnam in 2025, describe treatment regimens applied in clinical practice, and analyze the trend of single-pill combination (SPC) therapy. A multicenter cross-sectional study was conducted on 3,197 hypertensive patients receiving outpatient treatment at seven healthcare facilities across different regions. Results showed that 54.5% of patients were treated with monotherapy, while 45.5% received combination therapy with two or more agents, including 17.9% using SPCs and 15.9% using free combinations. Triple therapy accounted for 9.8% and quadruple or more for 1.7%. The proportions of patients achieving target blood pressure < 140/90mmHg and < 130/80mmHg were 73.6% and 26.5%, respectively. The use of SPCs was more common in provincial and central hospitals than in primary care settings. These findings indicate a positive shift in hypertension management in Vietnam, with increasing adoption of combination therapy, particularly SPCs, consistent with international recommendations (ISH 2020, ESH 2023, WHO 2025). However, the implementation remains heterogeneous, underscoring the need for standardized prescribing guidance and enhanced training to improve blood pressure control nationwide.
