Evaluation of surgical outcomes of medial canthoplasty using the Y-V technique for the treatment of epicanthal fold

Abstract

A prospective, single-arm interventional clinical study was conducted on 62 eyes undergoing medial canthoplasty using the Y-V technique for the treatment of epicanthal folds at the Vietnam National Eye Hospital from October 2023 to July 2025. Clinical parameters assessed before and after surgery included the severity of the epicanthal fold, palpebral fissure length, inner canthal distance, the ratio of inner canthal distance to palpebral fissure length, and intercanthal asymmetry. The results showed that 96.8% of eyes achieved complete resolution of the epicanthal fold immediately after surgery and remained stable during the first postoperative month; after 3 months, only 12.9% of eyes exhibited mild residual epicanthal folds. Palpebral fissure length increased and inner canthal distance decreased significantly, with a marked improvement in the inner canthal distance–to–palpebral fissure length ratio. Medial canthoplasty using the Y-V technique is proven to be an effective procedure that provides significant functional and aesthetic improvement in the treatment of epicanthal folds.
