Abstract

This study aimed to describe surgical outcomes of acute appendicitis in adults and to compare short-term outcomes between patients with complicated and uncomplicated appendicitis at Nhan dan Gia Dinh Hospital. In this observational study, 496 patients who underwent emergency appendectomy with preoperative computed tomography, intraoperative documentation, and histopathological confirmation of appendicitis were included. The primary outcomes were length of hospital stay, 30-day postoperative complications, Clavien–Dindo grade ≥ 2 complications, and 30-day readmission. Complicated appendicitis was identified in 200 of 496 patients (40.3%) and required longer hospital stay; the median length of stay was 3.99 vs 2.05 days; p < 0.001, and the readmission rate was also higher (6.0% vs 2.0%; p = 0.027). After multivariable adjustment, complicated appendicitis remained independently associated with prolonged hospital stay (mean ratio [MR] = 1.730; 95% confidence interval [CI]: 1.601 – 1.869; p < 0.001) and increased 30-day readmission (odds ratio [OR] = 2.988; 95% CI: 1.143 – 8.525; p = 0.025), but was not significantly associated with postoperative complications. Complicated appendicitis significantly increases inpatient resource utilization and the risk of 30-day readmission, highlighting the need for risk-stratified postoperative care, early recognition of warning signs, and closer follow-up after discharge.
