Value of extended hematological parameters in predicting the severity of non-mechanical acute pancreatitis

Abstract
Early prediction of severity in acute pancreatitis (AP) plays an important role in therapeutic strategies. A retrospective cohort study was conducted on 108 AP patients with non-mechanical etiologies at the time of admission. The prognostic value of extended hematological parameters (Neut, NLR, PLR, HFLC%, MicroR, IG%…) was evaluated via Receiver Operating Characteristic (ROC) curves and Youden’s index. NLR and MicroR showed statistically significant differences between severe AP group (n = 20) and non-severe AP group (n = 88) with p < 0.05. ROC analysis showed that NLR had the best prognostic value for severity with AUC = 0.709 (95% CI: 0.587 - 0.831; p = 0.004). At a cut-off > 5.66, NLR predicted severe progression with a sensitivity of 70.0% and a specificity of 61.4%. NLR and Neut both had high negative predictive values, 90% and 93.2%, respectively. NLR and Neut effectively support clinicians in risk stratification and optimizing the management of acute pancreatitis patients.
