Bacterial etiology and antibiotic resistance patterns in hospitalized patients with acute exacerbations of chronic obstructive pulmonary disease

Abstract

To characterize the pathogenic bacteria and antibiotic resistance patterns among hospitalized patients with acute exacerbations of chronic obstructive pulmonary disease (COPD) a retrospective descriptive cross-sectional study was conducted on 151 in-patients at the Department of Respiratory Medicine, Military Hospital 175, from October 2024 to December 2025. 24.5% of patients had positive bacterial cultures. Gram-negative bacteria predominated, with the most common isolates being Acinetobacter baumannii (37.8%), Klebsiella pneumoniae (32.4%), and Pseudomonas aeruginosa (21.6%). The isolated strains demonstrated high rates of antibiotic resistance, particularly to beta-lactams, quinolones, and carbapenems. A. baumannii exhibited the highest level of resistance, whereas K. pneumoniae and P. aeruginosa remained susceptible to selected antibiotics, including amikacin, ceftazidime/avibactam, ceftazidime, and cefepime. Multidrug-resistant Gram-negative bacteria are the leading pathogens associated with hospitalized acute exacerbations of COPD. The high prevalence of antibiotic resistance presents a major challenge for empirical antibiotic therapy and underscores the importance of local microbiological surveillance to optimize treatment strategies.
