Secondary cleft rhinoplasty using autologous costal cartilage: Improvement in nasal tip projection and rotation

Abstract
Secondary nasal deformity in patients with unilateral cleft lip is commonly characterized by inadequate nasal tip projection, decreased tip rotation, and nasal asymmetry. This case series included 20 patients with unilateral cleft lip who underwent rhinoplasty using autologous costal cartilage at 108 Military Central Hospital from June 2024 to December 2025. Standardized 90° lateral facial photographs obtained preoperatively and at 6 months postoperatively were analyzed using ImageJ software; we evaluated nasal tip projection based on absolute projection values and the Goode ratio, as well as nasal tip rotation based on the nasolabial angle. The results demonstrated significant improvement in both nasal tip projection and rotation after surgery. The study results confirm that rhinoplasty using autologous costal cartilage provides enhanced structural support and improves aesthetic outcomes in patients with unilateral cleft lip.
