Xác định đột biến gen SLC22A5 và phân tích phả hệ ở các gia đình có trẻ sơ sinh nghi ngờ thiếu hụt Carnitine nguyên phát

Thiếu hụt carnitine nguyên phát (Primary Carnitin deficiency - PCD) là một rối loạn di truyền gen lặn trên nhiễm sắc thể thường do đột biến gen SLC22A5, mã hóa cho chất vận chuyển cation hữu cơ 2 (OCTN2), gây giảm nồng độ carnitine trong huyết tương. Hiện nay, chẩn đoán PCD thường được dựa trên xét nghiệm hóa sinh kết hợp biểu hiện lâm sàng chưa thực sự đặc hiệu so với các bệnh lý khác. Từ những lý do trên, chúng tôi tiến hành nghiên cứu xác định các đột biến gen SLC22A5 cho 4 gia đình với 4 bệnh nhân có kết quả sàng lọc MS/MS nghi ngờ thiếu hụt carnitine nguyên phát, và 11 thành viên gia đình có mối quan hệ huyết thống bậc 1 với bệnh nhân. Bằng phương pháp giải trình tự Sanger đã xác định được 03 đột biến gen SLC22A5 gồm c.760C>T (p.Arg254*), c.51C>G (p.Phe17Leu) và c.338G>A (p.Cys113Tyr). Phân tích phả hệ cho thấy các đột biến có tính chất di truyền trong gia đình và phát hiện được các trường hợp người lành mang gen bệnh. Nghiên cứu khẳng định vai trò của xét nghiệm sinh học phân tử trong chẩn đoán xác định PCD và hỗ trợ tư vấn di truyền cho các gia đình có nguy cơ.
