4. Ý tưởng tự sát và một số yếu tố liên quan trên bệnh nhân rối loạn trầm cảm chủ yếu

Tự sát là vấn đề quan trọng và có tỉ lệ cao trên bệnh nhân rối loạn trầm cảm chủ yếu. Chúng tôi thực hiện nghiên cứu mô tả cắt ngang trên 151 bệnh nhân được chẩn đoán xác định rối loạn trầm cảm chủ yếu tại phòng khám Tâm thần kinh, Bệnh viện Đại học Y dược Thành phố Hồ Chí Minh trong thời gian từ tháng 3/2023 đến tháng 10/2023 với mục tiêu xác định tỉ lệ và một số yếu tố liên quan đến ý tưởng tự sát ở bệnh nhân rối loạn trầm cảm chủ yếu. Sau thời gian 7 tháng, chúng tôi thu được một số kết quả như sau: Đa số bệnh nhân là nữ (78,8%), không có tôn giáo (49,0%), ở thành thị (62,9%), học vấn từ trung học cơ sở trở xuống (53,6%), đã kết hôn (57,0%) và sống với vợ chồng/bạn đời (58,9%). Gần 80% bệnh nhân hiện ở giai đoạn trầm cảm nặng. Có mối liên quan giữa ý tưởng tự sát trong đời của bệnh nhân với trình độ học vấn (OR = 2,5; 95% KTC: 1,2 - 5,2), tình trạng hôn nhân (OR = 0,2; 95% KTC: 0,1 - 0,5), người sống chung (OR = 0,4; 95% KTC: 0,2 - 0,8), tiền căn bệnh đồng mắc (OR = 0,4; 95% KTC: 0,2 - 0,8), cảm giác tội lỗi (OR = 5,1; 95% KTC: 2,4 - 11,0), mức độ nặng của giai đoạn trầm cảm (OR = 11,0; 95% KTC: 2,2 - 55,8). Không tìm thấy mối liên quan giữa giới tính, công việc, tiền căn gia đình mắc rối loạn tâm thần hoặc tự sát và các triệu chứng lâm sàng khác với ý tưởng tự sát.
