Viêm gan tự miễn khởi phát sau nhiễm Epstein-Barr virus: Báo cáo ca bệnh

 Epstein - Barr virus (EBV) là một nguyên nhân gây viêm gan, mức độ viêm gan thường nhẹ và thoáng qua.
Ngoài ra, EBV còn là tác nhân quan trọng có khả năng gây khởi phát viêm gan tự miễn. Chúng tôi báo cáo một
trường hợp trẻ nữ, tuổi thiếu niên được chẩn đoán viêm gan do EBV. Sau khi điều trị bằng thuốc kháng virus
thì tình trạng transaminase máu vẫn tăng cao, Immunoglobulin G (IgG) tăng cao kéo dài, kháng thể kháng cơ
trơn (LC-1) trở nên dương tính và sinh thiết gan cho thấy hình ảnh điển hình của viêm gan tự miễn. Trẻ được
chẩn đoán viêm gan tự miễn type 2 khởi phát sau nhiễm EBV. Kết luận: Ở trẻ nhiễm EBV mà có biểu hiện
viêm gan kéo dài cần loại trừ viêm gan do các căn nguyên khác. Khi xuất hiện các tự kháng thể đặc hiệu với
viêm gan tự miễn nên cân nhắc sinh thiết gan và phối hợp các xét nghiệm để chẩn đoán viêm gan tự miễn.
