31. Surgical treatment of scar constracture of the finger by Z-plasty

Abstract

This study assessed the results of Z-plasty for the treatment of scar constructure of finger. A non-randomized controlled interventional study was performed on 18 patients with 36 scars constructure of finger at Hanoi Medical University Hospital and Bach Mai Hospital from 2020 to 2022. Z-plasty was performed on 31 fingers with linear band constructure, types I, II according to Rei Ogawa scar classification and Z-plasty combined with skin graft was performed on 5 fingers with broadband contracture < 1/4 circumferences type IIIa. At hospital discharge, all Z-flaps were alive, 1/36 of the fingers were infected, and the total active range of motion (TAM angle) were increased to 48.5 degrees (93.1% of the deficit). At 6 months after surgery, the average increase in TAM angle was 39.6 degrees (76%). Small flat scars appear in 30/36 fingers, 6 fingers have hypertrophic scars with increased pain sensation. The study showed that Z-plasty is useful with linear band type I and II scars according to Rei Ogawa's scar classification. With type IIIa, it is necessary to combine Z-plasty with skin grafting.
