18. Medial sural artery perforator flap in reconstruction of soft tissue defect in Ha Noi Medical University Hospital

Abstract

This study evaluates the results of covering soft tissue defects with medial sural artery perforator free flap. This is a non-randomised interventional study conducted among 13 cases of defects exposing tendons, bones, blood vessels or nerves. The wound sites were covered with medial sural artery perforator free flap. The effectiveness of defect coverage and the impact of the donor site were evaluated at the time of hospital discharge and 3 months after surgery. The flap used for coverage had an average size of 5,9 x 11,2cm², thickness of 10,2mm, pedicle length of 10,3cm, and each flap had an average of 1.9 perforating vessels. Most (12/13) flaps survived well, completely covering the defect, and 12/13 the donor site was closed directly. After 3 months, good limb function was observed in 9/12 patients, 2/12 had limited function due to severe primary injury despite good imaging results, 8/12 of the flaps were flat with the surrounding organization. All of the donor legs showed normal function, although 7/12 had hypertrophic scars but it was not a concern to the patient. The medial sural artery perforator flap has the advantage of being thin and less destructive to the donor site, thus is a good material for coverage in small to medium soft tissue defects.
