46. Spinal subdural hematoma: A case report and review

Abstract

Spinal subdural hematoma (sSDH) is a rare condition, causing neurological sequelae and significant mortality if not diagnosed and treated promptly. In this report, we present a case of a 77-year-old male who was being treated for a stroke with antiplatelet drugs and was diagnosed with idiopathic MTD with symptoms of acute back pain accompanied by weakness in both lower limbs and involuntary urination. Magnetic resonance injection of contrast agent for images of structures located in the spinal canal at the level of L4 to S1, size 60x12x12mm, increased signal on T1W and T1FS, decreased signal on T2W and STIR, no enhancement after injection on follow-up hematoma in the spinal canal. Posterior laminectomy decompresses the L4-L5 position, widely opens the hematoma capsule, drains the hematoma, stops the bleeding point, and releases the nerve roots. With timely diagnosis and treatment, the patient recovered significantly immediately after surgery and one month after surgery.
