28. Outcomes of laparoscopy in the management of upper urinary tract urothelial carcinoma at Hanoi Medical University Hospital

Abstract

The purpose of this study is to evaluate the effectiveness, safety and outcomes of laparoscopy and transperitoneal laparoscopy (TL) in the treatment of upper tract urothelial carcinoma (UTUC). The study included 46 patients, of which 40 patients underwent retroperitoneal laparoscopy (RL) and 6 patients underwent TL. The average operative time was 151.2 ± 33.5 minutes (Min 95; Max 330), of which the TL group had longer operative time than the RL group. The blood transfusion rate was 8.7% and all were in the RL group. All complications were grade I, II representing 10.8%, 6.5% respectively, and no complication was recorded in the TL group. Postoperative hospital stay was 7.2 ± 1.8 days (Min 6; Max 14), of which the TL group had a shorter hospital stay than the RL group. The mean follow-up was 19.0 ± 11.9 months (Min 6; Max 44), of which the disease specific mortality was 6.5%, 8.7% had local and bladder recurence, and 8.7% had distant metastasis. The mean follow-up of the TL group was 8.7 ± 2.9 months (Min 6; Max 14), no local or bladder recurrence was recorded. One patient had distant metastasis and died 5 months after surgery. Thus, laparoscopy and TL is a safe and effective procedure in the treatment of UTUC. Oncological outcomes obtained from our study are quite good. However, studies with larger sample sizes and longer follow-up are needed to establish definitive conclusions.
