Accuracy of PSPW index on 24-hour pH-impedance monitoring in the diagnosis of gastroesophageal reflux disease

Abstract
Our study aimed to evaluate the accuracy of PSPW index on esophageal pH-impedance monitoring in the diagnosis of GERD among Vietnamese patients having suspected reflux symptoms. A cross-sectional descriptive study was conducted among patients having suspected reflux symptoms and underwent 24-hour pH-impedance monitoring at Hoang Long Clinic and the Institute of Gastroenterology and Hepatobillary between March 2020 and February 2023. 112 patients were eligible, the mean age was 45.2 ± 11.9. 34 patients satisfied the diagnostic criteria of GERD, 24 patients had ERD and 10 patients had NERD. There was a significant difference in 24-hour pH impedance metrics between the GERD and non-GERD groups (p < 0.001). PSPW index correlated with Demeester, MNBI, AET scores (p < 0.001) and number of acid refluxes (p = 0.02). The area under the ROC curve (AUC) of PSPW index in GERD diagnosis was 0.72, with sensitivity of 50% and specificity of 83.3%, the optimal cut off value was 22%.
