40. Ocular complications after filler injection – a rare case report and literature review

Abstract

Ocular complications following filler injection, though rare, can result in severe consequences, the most devastating being blindness due to ophthalmic artery occlusion. This occurs when substances such as hyaluronic acid (HA) or autologous fat are injected into highly vascularized regions like the forehead and nose, leading to embolism or vasospasm. Other complications include anterior segment ischemia syndrome, cerebral infarction, and phthisis bulbi. This report presents a case of a male patient admitted to Hanoi Medical University Hospital (HMUH) with sudden vision loss and right eyelid ptosis 30 minutes after HA injection into the forehead. Clinical evaluation revealed central retinal artery occlusion, anterior chamber hemorrhage, corneal edema, and cerebral infarction. The patient underwent emergency management involving acetazolamide infusion, intra-arterial hyaluronidase injection under digital subtraction angiography (DSA) guidance, intraocular pressure reduction, and anti-inflammatory treatment. After seven days of hospitalization, ocular motility and ptosis significantly improved, but visual recovery remained limited. The patient was discharged with continued outpatient treatment and scheduled follow-ups to monitor potential complications.
