12. The correlations between serum Ferritin levels and mortality among pediatric septic shock patients

Abstract
This cross-sectional study included 114 children with septic shock admitted to the Pediatric Intensive Care Unit (PICU) of Vietnam National Children’s Hospital between March and November 2023. The objective was to assess the correlation between serum ferritin levels and mortality of pediatric septic shock. Results revealed significantly elevated serum ferritin levels in septic shock patients, with a median of 642.0 ng/mL (IQR: 339.0 – 1976.8). Non-survivors exhibited much higher ferritin levels (median 1899. 5 ng/mL, IQR: 934.5 – 4278.8) compared to survivors (median 176.0 ng/mL, IQR: 101.4 –5289.5). The area under the ROC curve (AUC) for ferritin levels in predicting mortality was 0.77 (95% CI: 0.67 – 0.88). Ferritin levels showed significant correlations with PRISM-III score (ρ = 0.46, p < 0.0001), pSOFA score (ρ = 0.42, p < 0.0001), lactate (ρ = 0.3, p = 0.001), and maximum VIS (ρ = 0.38, p < 0.0001). Logistic regression analysis identified ferritin levels > 1369.5 ng/mL as an independent predictor of hospital mortality. These findings suggest that serum ferritin is a valuable prognostic biomarker in pediatric septic shock, particularly when measured within the first 24 hours of PICU admission.
