50. Outcomes of intermediate-type anorectal malformations in male using posterior sagittal approach with sphincter preservation at the Vietnam National Children’s Hospital, 2020 - 2023

Abstract
Anorectal malformations are common in children, with multiple treatment options available; however, no optimal method has yet been established for pediatric patients. This study aimed to evaluate the outcomes of posterior sagittal anorectoplasty (PSARP) with sphincter preservation for intermediate-type anorectal malformations in male children at the Vietnam National Children’s Hospital from January 2020 to December 2023. A retrospective descriptive study was conducted on 43 male patients diagnosed with intermediate-type anorectal malformations who underwent PSARP with sphincter preservation at the Department of Gastrointestinal Surgery – Center for General Surgery, Vietnam National Children’s Hospital, during the above period. The mean operative time in the study group was 77.33 ± 27.68 minutes. The intraoperative fistula detection rate was 46.5%. The mean postoperative hospital stay was 5.7 ± 2.7 days. Follow-up was achieved in 35 out of 43 operated patients, with a mean follow-up duration of 38.9 ± 15.3 months. The most common bowel movement frequency was 1 - 2 times/day (65.7%), with constipation occurring in 3 patients (8.6%). The rate of fecal soiling was 51.4%. Complete continence was achieved in 82.6% of patients, while 17.4% had partial continence. Good postoperative bowel function was observed in 71.4% of patients, fair function in 22.9%, and poor function in 5.7%. The most frequent postoperative complication was rectal mucosal prolapse (28.6%). PSARP with sphincter preservation for intermediate-type anorectal malformations in male children is a safe and effective treatment method.
