A multidimensional diagnostic approach to neuroblastoma from a case of recurrent joint pain in a child

Abstract
Neuroblastoma is one of the most common solid tumors in children under five years of age, yet it is frequently diagnosed late due to its nonspecific clinical manifestations. We report the case of a 35-month-old boy who presented with recurrent fever and ankle pain over a 4-month period. Initial treatment was directed toward common causes of inflammatory arthritis. However, a structured approach involving close monitoring of clinical progression and repeat laboratory testing revealed transient hematologic abnormalities and persistent symptoms unresponsive to standard treatment, raising suspicion for an underlying malignancy. Bone marrow aspiration, immunohistochemistry, and imaging confirmed the diagnosis of metastatic neuroblastoma with bone marrow involvement. This case highlights the importance of maintaining appropriate clinical suspicion and adopting a multidimensional approach when evaluating atypical joint symptoms in young children. Timely reassessment and diagnostic expansion are critical for early detection of malignant diseases and improving patient outcomes.
