Acute Calcium Pyrophosphate deposition disease following parathyroidectomy: A case report

Abstract
Acute calcium pyrophosphate deposition (CPPD) disease is an inflammatory arthropathy characterized by acute synovitis resulting from the deposition of calcium pyrophosphate crystals, typically associated with disturbances in calcium–phosphate metabolism. Acute CPPD occurring after parathyroidectomy is rare and has been reported in the literature only in a limited number of case reports. In this article, we report a clinical case of acute CPPD developing after parathyroidectomy at Phenikaa University Hospital. Notably, this represents the first reported case of acute CPPD involving the ankle–foot joint following parathyroidectomy in a patient with type 2 diabetes mellitus. The coexistence of type 2 diabetes mellitus complicated the clinical presentation and posed a significant diagnostic challenge. This case highlights the importance of thorough assessment of surgical history and systematic exclusion of alternative diagnoses in the differential diagnosis, which are essential for establishing an accurate diagnosis and selecting an appropriate treatment strategy.
