Mortality predictors in elderly patients undergoing left main coronary artery percutaneous coronary intervention

Abstract
Left main coronary artery (LMCA) disease in elderly patients presents a significant clinical challenge due to complex anatomical lesions and a high burden of comorbidities. Data regarding long-term outcomes following intervention in this population remain limited in Vietnam. This study aimed to identify prognostic factors for long-term mortality in elderly patients undergoing LMCA percutaneous coronary intervention (PCI). We conducted a retrospective longitudinal cohort study involving 116 patients aged 60 years old who underwent LMCA PCI at Cho Ray Hospital between July 1, 2019, and June 30, 2020. Follow-up data were obtained via telephone interviews after five years. The 5-year all-cause mortality rate was 31.9%, and the 5-year survival probability was 68.1%. In the adjusted multivariate Cox regression analysis, independent predictors of 5-year mortality included age (70 years old) (HR = 3.4), cardiogenic shock on admission (HR = 3.2), elevated CK-MB levels (HR = 1.1 per 50 U/L increase), three-vessel disease (HR = 2.2), and ostial LMCA lesions (HR = 2.6).
