Outcomes of transoral endoscopic thyroidectomy vestibular apporach in patients with bilateral papillary thyroid carcinoma at Hanoi Medical University Hospital

Abstract

Transoral endoscopic thyroidectomy vestibular approach (TOETVA) is a minimally invasive technique for the treatment of thyroid cancer. However, data on its safety and efficacy in bilateral papillary thyroid carcinoma remain limited. This retrospective and prospective descriptive study was conducted at Hanoi Medical University Hospital from January 2020 to July 2024. Thirty patients with bilateral papillary thyroid carcinoma underwent TOETVA total thyroidectomy with bilateral central neck dissection; risk stratification and indications for radioactive iodine therapy were based on the 2015 American Thyroid Association guidelines. Most patients were female (96.7%), and the mean age was 33,1 ± 8,6 (16 - 51) years old. The mean operative time was 103.8 ± 31.1 (65 - 190) minutes, and the mean blood loss was 16,3 ± 8,7 (5 - 40) mL. Temporary complications included hypoparathyroidism in 10.0% and recurrent laryngeal nerve injury in 13.3%; no permanent complication was recorded. 46.7% developped occult central lymph node metastasis. At 3 months, neck ultrasonography showed no residual disease in any patient; four cases with abnormal thyroglobulin and/or anti-thyroglobulin antibody levels normalized after 6 – 9 months. TOETVA is a safe, effective, and cosmetically favorable approach for selected patients with bilateral papillary thyroid carcinoma.
