Diagnostic performance of colposcopy and associated factors in detecting CIN2+ lesions at Hanoi Medical University Hospital

Abstract
This cross-sectional study included 486 patients who underwent colposcopy and cervical biopsy at the Department of Obstetrics and Gynecology, Hanoi Medical University Hospital; we evaluated the predictive value of colposcopy and related factors for CIN2+ lesions. Most women were from 31 to 50 years old, had given birth at least once, and had a low history of abortion/ miscarriage/ curettage procedures (< 3 times). Clinical symptoms were generally subtle and nonspecific. Screening tests using cytology and HPV-DNA showed limitations in detecting CIN2+, with low sensitivity for cytology (65.9%) and very low specificity for HPV-DNA (3%). Factors associated with cervical precancerous and cancerous lesions included colposcopic findings, HPV types 16/18, cytological abnormalities, and age ≥ 45 years old. In contrast, obstetric history, the other 12 high-risk HPV types, and multiple HPV infections were not significantly associated with cervical lesions.
