Mất trũng huyết áp và một số yếu tố liên quan ở bệnh nhân tăng huyết áp điều trị nội trú tại Bệnh viện Hữu Nghị

Tăng huyết áp là gánh nặng y tế toàn cầu và là nguyên nhân hàng đầu gây tử vong và tàn phế. Trong đó, mất trũng huyết áp được xem là dấu hiệu tiên lượng xấu đối với các biến cố tim mạch và tử vong. Nghiên cứu mô tả cắt ngang được thực hiện trên 110 bệnh nhân tăng huyết áp điều trị nội trú tại khoa Nội tổng hợp A, bệnh viện Hữu Nghị từ tháng 1/2023 đến tháng 9/2023 nhằm xác định tỷ lệ mất trũng huyết áp và các yếu tố liên quan. Tất cả bệnh nhân được đo huyết áp lưu động trong 24 giờ và phân loại theo tiêu chuẩn ESH 2023. Các yếu tố được khảo sát gồm tuổi, giới, chỉ số khối cơ thể, rối loạn lipid máu, đái tháo đường, bệnh thận mạn, bệnh mạch vành, tiền sử tai biến mạch não và thời điểm dùng thuốc hạ huyết áp. Kết quả cho thấy tuổi trung bình của nhóm nghiên cứu là 77,1 ± 8,9 tuổi, tỷ lệ mất trũng huyết áp rất cao 82,7%. Bệnh nhân trên 70 tuổi có liên quan có ý nghĩa thống kê đến tình trạng mất trũng huyết áp, trong khi các yếu tố khác như đái tháo đường, bệnh thận mạn, rối loạn chuyển hoá lipit, uống thuốc 1 lần buổi sáng có xu hướng làm tăng nguy cơ mất trũng huyết áp.
