Airway resistance characteristics in pediatric patients with adenoid hypertrophy requiring surgery

Abstract
This prospective study evaluated airway resistance using impulse oscillometry (IOS) before and one month after adenoidectomy in 63 children with adenoid hypertrophy at Hanoi Children's Hospital (July 2025 – February 2026). The mean age was 7.7 ± 2.6 years old. Preoperatively (n=63), the prevalence of airway obstruction was 23.8%, which was significantly higher in the grade 3 hypertrophy group (61.5%) compared to the grade 2 group (14.0%) (p = 0.001). One month post-adenoidectomy (n = 36), IOS parameters improved significantly: R5, R20, R5–R20, AX, and Fres decreased, whereas X5 and X20 increased. Normal respiratory function rose from 58.3% to 91.7%, with complete resolution of moderate to severe obstruction. Adenoidectomy significantly improves respiratory mechanics and alleviates airway obstruction. Concurrently, these results confirm that IOS is a non-invasive and objective method for monitoring pediatric respiratory function.
