Clinical, paraclinical characteristics and preliminary results of diffuse large B-cell lymphoma treatment in elderly patients at K Hospital

Abstract
This study aimed to describe the clinical and paraclinical characteristics, as well as the preliminary treatment outcomes of diffuse large B-cell lymphoma (DLBCL) in elderly patients. A descriptive study was conducted on 124 patients aged 70 years and older with CD20-positive DLBCL, who received first-line treatment with Rituximab-based regimens (combined with CHOP, miniCHOP, CVP, or GCVP) at K Hospital between 2019 and 2024. Following 6 to 8 cycles of treatment, the complete response (CR) rate and overall response rate (ORR) were 59.7% and 73.4%, respectively. A significant difference in efficacy was observed among the regimens: the R-CHOP regimen achieved a CR rate of 69.3% and an ORR of 80.0%, which were significantly higher than those of the R-miniCHOP regimen (CR: 46.7%, ORR: 64.5%; p = 0.024). Furthermore, heart failure induced by the cumulative toxicity of anthracyclines remained a major barrier in this elderly population. Therefore, close monitoring of cardiac function through periodic echocardiography, alongside quantitative measurements of Troponin T and proBNP, is essential for the early detection of subclinical myocardial injury.
